Division of Meridian and Structural Medicine, School of Korean Medicine, Pusan National University, Yangsan, Korea In the present study, the analgesic effects of scolopendrid pharmacopuncture (SP) on the neuropathic pain in rats and on the low back pain in herniated lumbar disk patients were examined. In animal study, neuropathic pain was induced by tight ligation of L5 spinal nerve. When rats developed pain behaviors, 100 ¥ìl of scolopendrid extract was injected into several acupuncture points under isoflurane anesthesia. The foot withdraw latency of the hind limb was measured for an indicator of pain level after each manipulation. The SP applied to ipsilateral BL23 point near injury site at a dose of 4 mg/kg increased the mechanical threshold of the foot significantly for the duration of 4 hr suggesting a partial alleviation of pain. The analgesic effect of the SP is equivalent to 50 mg/kg of gabapentin injection. The data suggest that SP produces a potent analgesic effect in a rat model of neuropathic pain. In the clinical study, 70 patients diagnosed with herniated disk were recruited. SP treatments were administered for 7 days and follow-up study was observed during 14 months. Acupuncture points in the bladder meridian, gallbladder meridian and governor vessel were used for ST treatment. Digital infrared thermal imaging (DITI) analysis, physical examinations, oswestry disability index and self-conscious pain test were evaluated. All of the outcome measurements were significantly improved during the follow-up period. The results suggest that the SP therapy is a useful option for the treatment of low back pain in herniated lumbar disk.
